How do I Connect to Google Healthcare API using SMART on FHIR and APIGEE

I've followed the steps here https://cloud.google.com/architecture/using-apigee-with-the-cloud-healthcare-api to connect google healthcare api to apigee, my question is how do i implement smart on fhir on apigee? There seems to not be any documentation to help with this.

For health APIX, is it available for free? I've tried to register an account on the portal and it hasn't been approved yet.

I think the demonstration site is "Free". but ... to be honest I don't know who administers that site or how to check on approvals or pending requests.
